Practice Area

We accept patients who live in the area highlighted on the Practice Area Map.

Catchment Area

To find out if your post code is within our catchment area- Please click on the map and press the search button in the top right corner. Enter your postcode and you will see where your address is in comparison to our catchment area.


If you would like to register as a patient, please:

  • Print out the form below and bring to reception with two forms of identification – one picture form (eg passport, driving licence) and one form for proof of address (eg utility bill dated in the last three months).
  • Please complete the details on the Practice Registration Form, including organ donation details if appropriate.
  • If you do not wish to have a Summary Care Record, please fill out an opt-out form and return to the surgery. For more information on the NHS Summary Care Records, please click here.

Completing the registration form will allow us to provide a service tailored to your medical requirements while we wait for full medical records to be sent to us from your former GP.

All information provided by you on the registration form is only used by the clinical staff at the practice and is confidential.

Patients Moving To an Area Outside Of Our Catchment Area

It is the practice policy that if registered patients move, from living within the practice area to outside the practice area, they will be de-registered from our practice list on the grounds that the patient has moved out of our area.

It is the patients responsibility to re-register at a new GP surgery in the area where they are now living in order to receive continuing care.
